RESOLUTION - Honoring Connie A. Farakhan on her retirement for 29 years of service to the Kansas City, Missouri Health Department.

WHEREAS Connie A. Farakhan completed her master's in psychology from the University of Missouri-Kansas City and bachelor's in psychology with a minor in biology from Seton Hall University in South Orange, New Jersey; and

WHEREAS, over a nearly 29-year career at the Kansas City, Missouri Health Department, Connie Farakhan has been recognized and has achieved the following: served as a representative for the Department's local Office of Minority Health, as Safety Trainer as part of the Health Communications Team, as Duty Officer for Emergency Response, as a Chronic Disease Trainer, as the liaison for the "Building a Healthier Heartland," a bi-state regional collaborative to reduce obesity, and as the Department liaison for the Community Transformation federal grant. She also assisted in regional and bi-state efforts that led to numerous policy changes for Kansas City, such as smoke free housing and parks, healthier vending and eating, increased physical activities in child care and schools, breast-feeding friendly worksites, and more; and

WHEREAS, Ms. Farakhan assisted with the Mayor's Minority Health Improvement Task Force and committees which led to the establishment of the Kansas City Health Commission. She was also a key staff member assisting with the Emergency HIV Declaration presented by the Health Department. Connie has worked ongoingly to inform organizations and the community about the social determinants of health and the need for equity and systems changes; and

WHEREAS Connie Farakhan has served the City of Kansas City, Missouri Health Department since 1995, and will retire from the position of Public Health Specialist III on August 1, 2024; NOW, THEREFORE,
